Partial Care

From young adults in transition, to seniors in need of specialized care, partial care programs provide adults living with a mental illness with the support needed to prevent psychiatric hospitalization and improve functioning in the community. Utilizing principles of wellness and recovery and peer support systems, we work with individuals to create a personalized wellness and recovery plan, develop positive relationships and improve overall quality of life.

Services Include:

  • Mental health, addiction and co-occurring services
  • Group and individual sessions
  • Psychiatric support
  • Nursing assessment and coordination with medical providers
  • Case management
  • Daily living skills groups
  • Medication monitoring
  • Pre-vocational training (Burlington County)
  • Tobacco cessation support

Eligibility:

  • Adults age 18 years and older
  • Burlington, Mercer or Camden County resident
  • Primary mental health diagnosis
