Oaks provides a full continuum of individualized services for children and adults living with a developmental disability in an encouraging, supportive environment. In-home, residential and day programs can help your loved one reach greater levels of independence throughout each stage of life.

We believe everyone deserves to be treated with dignity and respect. Our professional, caring staff utilize Trauma Informed Care and value strong connections with family, peers, school systems and community organizations. Programs are designed to allow individuals to reach their full potential and improve quality of life for the entire family.

Bridging Family Support provides intensive in-home supports for individuals of all ages with developmental/intellectual disabilities and their families. The program is a lifeline for families caring for a loved one challenged with communication, social interaction and behavior difficulties. The Pat LeBon Day and Empowerment Programs support adults dually diagnosed with a mental illness and developmental disability. The program revolves around the recurring events of daily living which presents opportunities including training perceptual abilities, coordination, communication skills, employment skills and increased independence with self-care/hygiene.

Oaks provides 24/7, caring residential services for children and adults with developmental disabilities. Our beautifully maintained, fully accessible group homes are located in safe residential neighborhoods. Individualized services include therapy, medical care, behavioral support, daily living skills development and socialization. Programs provide independent living for adults ready to live in the community with limited support. Individuals receive assistance with transportation, nutrition, shopping, budgeting, medication administration and community involvement. Staff are available 24 hours a day, 7 days a week.

Transition to Adult Services

When your child turns 21, Oaks will walk hand and hand with you through the transition process. Our full continuum of services for adults living with developmental disabilities includes day programs, supported living and 24/7 residential services.

Oaks works closely with the Department of Developmental Disabilities (DDD). Our goal is to help families navigate the system of care and find the right fit for their loved one. To determine eligibility and make a referral, call your identified DDD Support Coordinator.
